When Balan’s maternal uncle, who suffers from mental illness, passes away, the superstitious village and Balan's own family subconsciously begin looking for the "next in line." After Balan experiences a nightmare and a brief period of insomnia brought on by stress, those around him instantly misinterpret his exhaustion as the onset of madness.

Thaniyavarthanam (which translates to "The Repetition" or "The Recurring Theme") tells the tragic story of Balan (played by Mammootty), a deeply loved and respected schoolteacher. Balan belongs to a traditional, matrilineal Hindu family in rural Kerala. However, the family harbors a dark superstition: they believe a ancestral curse dictates that one male member of every generation must lose their sanity.
